Requirements for Biometric Passports

Issuance of First-Time Passport (adults)

1.    Fingerprints form click here and Passport Renewal form click here. Write your first and last names exactly as they appear in your legal documents (blue or black ink). NOTE: DO NOT NOTARIZE FINGERPRINTS

  • Please make sure that the written information is legible and in clear block letters.

2.    Two (2) Copies of a recent birth certificate with barcode (issued on or after 2010). If you do not have a recent birth certificate, please click here for requirements.
3.    Cédula of Identity and Electoral (ONLY CEDULAS RENEWED AFTER 2014 WILL BE ACCEPTED). 
4.    Copy of a picture ID (examples: driver’s license, school ID, green card, etc.) IF YOU DO NOT HAVE A CEDULA OF IDENTITY AND ELECTORAL PLEASE PROVIDE 2 FORMS OF ID'S
5.    Proof of Address
6.    Notarized affidavit stating that person has never been issued a Dominican passport. Click here for example of affidavit. Cost for this service is $50.00.
7.    Two (2) 2x2 passport photographs. Please print applicant's name on the back of each picture.

  • Please write full name on the back of each photo. Do not staple the photos to the Passport Renewal form (indicated on item 1). The photos must be sent in an envelope along with other documents.
  • In all pictures: Both ears must be visible and bangs must be pulled away from forehead and face. Please do not smile or wear eyeglasses or any jewelry (earrings, studs, necklaces, piercings, etc.). You must also wear shirts/tops/dresses with sleeves and collars.

8.    Costs: $180.00 (passport) + $50.00 (notarized affidavit) = $230. Please send money order in the amount $230.00 payable to the order of the Dominican Consulate.
9.    A SASE (pre-addressed and pre-paid) envelope to return the renewed passport.  We recommend that you use the services of an overnight courier (FedEx, UPS, DHL, etc.). Note, if using FedEx we are not responsible for shipping payments.

3.    Mother’s or father’s Cédula of Identity and Electoral (ONLY CEDULAS RENEWED AFTER 2014 WILL BE ACCEPTED) or current passport.

  • Note: If minor child does not reside with parents, parents must provide a notarized affidavit authorizing the minor’s legal guardians to complete the passport process. If minor does not live with both parents, the noncustodial parent must grant permission (a notarized Affidavit) for the child to be issued a passport.

  • Note: For name change, such as from single to married or vice versa, send a document confirming your civil status (i.e., marriage certificate, divorce decree, etc.). There is an additional $50.00 fee for this service.

Issuance of Passport Due to Loss

6.    A notarized affidavit requesting the issuance of a passport due to loss (sample affidavit). Cost for service is $50.00.
